I use a client portal for scheduling appointments. If you are a new patient, you may view my availability and contact me by phone or email to request an appointment. If you are an existing client, you may use your portal account to request appointments.
All clients will be given access to the portal by the end of their first appointment. Please contact me if you lost your password to your portal account.
I ask that you complete a few brief forms prior to our first appointment. These are available by clicking the link below or I will leave a blank copy in the waiting area before your appointment time at your request.
I also recommend that you review the Practice Agreement and Notice of Privacy Policies (also available by clicking below) prior to our first appointment. We will discuss these documents and I will answer any questions or concerns before we begin our work together.
