I use a client portal for scheduling appointments. If you are a new patient, you may view my availability and contact me by phone or email to request an appointment. If you are an existing client, you may use your portal account to request appointments, complete necessary paperwork, and update payment information.
NOTE: FRIDAY APPOINTMENTS ARE EXCLUSIVELY TELEHEALTH. APPOINTMENTS ON MONDAY/TUESDAY/WEDNESDAY/THURSDAY MAY BE TELEHEALTH OR IN-PERSON AT YOUR REQUEST
All intake paperwork, payment information, and other required forms will be shared with you directly via the patient portal. I ask that you review and complete all necessary paperwork prior to your first appointment and as new forms are added during our work together. Please contact me with any questions or concerns about the forms shared with you.
